Curis Diagnostic Solutions, LLC offers mobile diagnostics and phlebotomy, DNA testing, drug and alcohol testing, DOT compliance services, fingerprinting, mobile notary, specimen transport, temporary-location services, facility partnerships, and an affiliates program.

Some services have special legal or compliance requirements such as:

  • HIPAA
  • DOT and OSHA chain-of-custody
  • State identity verification and fingerprinting rules
  • Court-ordered compliance
  • Notary and document verification standards

By scheduling or receiving services, you agree to comply with any requirements associated with those services.

3. Eligibility to Use Our Services

You must be:

  • At least 18 years old, or
  • A minor with valid parental or guardian consent

You must provide accurate, complete information. You are responsible for ensuring you are legally allowed to use the service you are requesting.

4. User Responsibilities

6. Payment Terms

  • Some services are nonrefundable once performed
  • Additional travel or compliance fees may apply

Chargebacks or fraudulent disputes may result in service refusal or legal action.

7. Compliance-Based Services

Some services require strict legal compliance. By using these services, you agree to the following:

DNA Testing

You must have the legal right to request testing and provide all required identification. Attempting fraudulent parentage, immigration, or legal testing requests is prohibited.

Drug and Alcohol Testing

You agree to comply with DOT, employer, or legal requirements, including valid identification and adherence to protocols.

Fingerprinting Services

You agree to follow state and federal identity rules. Submission to FBI, FINRA, or other agencies may carry additional requirements.

Notary Services

You must provide identification acceptable under state notary law.

8. Website Content and Intellectual Property

All website materials such as text, graphics, logos, and service descriptions belong to Curis Diagnostic Solutions, LLC or licensed third parties.

You may not:

  • Modify

17. Digital Download Policy

Curis Diagnostic Solutions, LLC

All Digital Download Sales Are Final

Once a digital product has been downloaded, all sales are final. Because digital items cannot be returned, exchanged, or “un-downloaded,” we do not provide refunds for completed downloads.

Download Limits

Each purchased digital product includes:

  • A maximum of 3 downloads
  • A 7-day access windowfrom the time of purchase

After download limits are reached or the access window expires, the product link will no longer be available.

Download Issues or File Errors

If you experience technical issues when downloading the product, or if the downloaded file is incomplete, corrupted, or incorrect, we will make every reasonable effort to assist you.

We will:

  • Verify your purchase
  • Reset your download limit if appropriate
  • Reissue the correct product file

We will replace the file, not refund the order.

Customer Responsibility

It is the customer’s responsibility to:

  • Ensure a stable internet connection during download
  • Save the downloaded file to a secure location
  • Complete the download within the allowed timeframe

Curis Diagnostic Solutions is not responsible for loss of downloaded files after successful delivery.

18. Return and Refund Policy for Physical (Non-Digital) Products

This policy applies only to physical products purchased from Curis Home Healthcare, LLC.
It does not apply to digital products, which are covered under the Digital Download Policy.

18.1 Returns for Unopened Products

We accept returns for unopened, unused, and undamaged products within 14 days of the delivery date.

To qualify for a return:

  • The product must be in its original, sealed packaging
  • The item must show no signs of use or tampering
  • You must provide proof of purchase

Once we receive and inspect the returned item, we will issue a refund to the original payment method, minus any shipping costs.

18.2 No Returns or Refunds for Opened Products

We do not accept returns or offer refunds for:

  • Opened products
  • Used products
  • Products with broken seals or missing packaging

For health, safety, and quality reasons, opened products cannot be restocked and are therefore not eligible for return or refund.

18.3 Return Shipping Costs

Customers are responsible for the cost of shipping returned items unless we shipped the wrong product or the product arrived damaged.

18.4 Damaged or Incorrect Products

If your product arrives damaged or incorrect:

  • Contact us within 7 daysof delivery
  • Provide photos and your order number

We will replace the item or issue a refund, depending on the situation.
