This job combines medical and non-medical duties, and the design employs RNs and LPNs who desire a flexible work schedule and those seeking to get out of inpatient hospital environments, providing skilled home care to a broader client base.

RNs and LPNs will be deployed to meet needs in private duty nursing support, care for medically complex clients, post-hospital care, or long-term in-home medical support.

 Summary

We’re seeking a skilled Medical Home Support Assistant to provide comprehensive in-home care for clients with medical needs. This role combines hands-on medical support with non-medical assistance to help clients remain safe, comfortable, and supported in their homes. The position works under the direction of a licensed nurse, physician, or care manager, depending on client needs and state regulations.

The ideal candidate is clinically competent, compassionate, dependable, and able to balance medical responsibilities with daily living support.

Medical Support Responsibilities

(Duties performed according to licensure, training, and state scope of practice)

  • Assist with skilled nursing care as directed by licensed providers
  • Monitor and document vital signs and changes in condition
  • Assist with medication administration or medication setup (as permitted)
  • Perform wound care, dressing changes, and skin integrity monitoring
  • Assist with catheter care, ostomy care, and incontinence management
  • Support post-operative or post-hospital recovery care
  • Assist with mobility, transfers, and the use of medical equipment
  • Support chronic condition management (diabetes, cardiac, respiratory, etc.).
  • Perform venipuncture and blood specimen collection as ordered
  • Initiate, monitor, and discontinue IV therapy as permitted by licensure
  • Monitor IV sites for complications and maintain IV line care
  • Observe, document, and promptly report changes in condition
  • Follow physician orders, care plans, and clinical protocols
  • Maintain infection control and safety standards

Non-Medical Support Responsibilities

  • Assist with activities of daily living (bathing, dressing, grooming, toileting)
  • Provide companionship and emotional support
  • Prepare meals and assist with feeding as needed
  • Perform light housekeeping related to client care
  • Assist with errands and transportation to appointments
  • Provide medication reminders when applicable
  • Maintain a clean, safe, and organized home environment
  • Support daily routines while promoting independence and dignity

Client Populations Served

  • Seniors with complex medical needs
  • Individuals with disabilities requiring skilled support
  • Post-surgical or post-hospital discharge clients
  • Long-term medically fragile clients
  • Private pay and agency-managed care clients

Qualifications

  • RN, LPN, MA, or equivalent medical home care experience (role dependent)
  • Current license or certification as required by position and state
  • Experience providing skilled in-home or facility-based care preferred
  • Strong understanding of infection control and patient safety
  • Ability to follow care plans and provider instructions accurately
  • Excellent communication and documentation skills
  • Compassionate, professional, and dependable

Certifications and Training (Preferred or Required Based on Role)

  • CPR / BLS
  • Medication administration training
  • Wound care training
  • Chronic disease management
  • Dementia or cognitive care training
  • Palliative or hospice care experience

Physical and Other Requirements

  • Ability to lift, reposition, and assist clients safely
  • Ability to stand, walk, bend, and perform hands-on care
  • Valid driver’s license and reliable transportation if required
  • Ability to pass background screening and drug testing


Schedule and Work Environment

  • Work performed in private homes
  • Flexible schedules based on client needs
  • Shifts may include days, evenings, weekends, or live-in care


Compensation and Benefits

  • Pay range $45 to $55 per hour, shift and pay differentials for RN and LPN Support, and performance bonuses.
